Mother’s Day Questionnaire Card

Hello there! Michelle from My Belle Michelle. And I’m so excited to share a fun Mother’s Day printable for you today. I love getting drawings from my kids, and I also love reading their answers they give to questionnaires “about mom”. So I decided this year to do a card for mom’s to open that combine the two! I think it turned out really cute, and it is something quick and easy for the kiddos to do.

My kids had a lot of fun filling out the questionnaire and coloring the card the way they liked.

Hope you enjoy it. I ended up taking the folding lines out of the card because varying printing, but you just print out the card file, and fold it in 4ths with the Happy Mother’s Day for the cover. Easy, peasy – and a perfect card to attach to a cute mother’s day gift.

  1. Thank you so much for sharing this! I was looking for something simple, yet meaningful for my students to make for their moms. This fits the bill perfectly!

  2. Just wanted to thank you for posting this. We’re already doing a painting project so I was trying to find a Mothers Day card that was creative and sentimental from kids but wasn’t a craft. This was exactly what I was looking for! Thanks!
